THE OFFICE OF VITAL STATISTICS DURING COVID-19 CRISIS

While the building is closed to the public, the Office of Vital Statistics is up and running. We are here to assist with the majority of our daily functions via mail, email or fax.  

In line with Governor Murphy’s directive, this office will be available on Tuesdays and Thursdays to process applications for marriage/civil unions BY APPOINTMENT ONLY. Click here to view the guide on how we will process said applications while our offices are closed to the public. 

You may continue to request certified copies of birth, marriage and death certificates by filling out the application and mailing it to us with the required proofs. The process for this is described in the next sections below.

MARRIAGE LICENSES:

  • Applications for marriage/civil union are taken when one or both parties reside in Bridgewater Township
  • Applications are by appointment only (908) 725-6300 ext. 5021
  • Both Parties must be present to make application for a marriage license
  • A witness, 18 years or older must be present & must have proper proof of Identity (Valid driver’s license/ Passport/ State/Federal ID

Required Documents when applying for a Marriage/Civil Union License

  • Proof of Identity:
    • Valid driver’s license/ Passport/ State/Federal ID
  • Proof of Residency:
    • Valid driver’s license / utility bill /  lease / tax return
  • Name and Birth place (country or state): of both sets of parents (maiden name of mother)
  • Name, address and phone number of the person performing the ceremony
  • Name of the place, and city where the ceremony will be held
  • $28.00 payable in cash, check, or money order
  • Death certificate or divorce papers, if applicable
  • Social Security Number

FREQUENTLY ASKED QUESTIONS

WHEN CAN I APPLY FOR A MARRIAGE LICENSE IN BRIDGEWATER TOWNSHIP?

  • If either party resides in the Bridgewater Township.
  • If neither party resides in the State of New Jersey but the event will be in Bridgewater Township then you must apply there and the license can only be used in Bridgewater Township.
  • A New Jersey marriage or civil union license may never be used outside of the State of New Jersey and a license issued in another State may never be used within the State of New Jersey.

WHAT TIME FRAMES MUST I ADHERE TO WHEN APPLYING FOR A MARRIAGE LICENSE?

  • The Registrar cannot issue a marriage or civil union license sooner than 72-hours after the application for a license has been made.
  • The application for marriage license is valid for six months from the date it is filed.
  • Once the license is issued, it is valid for 30-days from the date of issuance.

WHERE SHOULD A MARRIAGE OR CIVIL UNION RECORD BE FILED?

  • The person performing the marriage or civil union must file the license with the Registrar of the municipality in which the ceremony was performed. The couple must ensure this is done as failure to do so may cause considerable inconvenience at a later date when proof of marriage certificate is needed.
  • The person performing the marriage/civil union has five (5) days to file the license with the Registrar of the municipality in which the ceremony was performed.

WHAT IS A REMARRIAGE?

  • For individuals who are currently married, the option exists to reaffirm your relationship/vows by registering a remarriage.
  • To apply for a remarriage, the couple must provide proof of their existing marriage & meet the aforementioned requirements for entering into a marriage in New Jersey.
  • The process for a remarriage license is the same as for the marriage license, except that the remarriage license must be accompanied by the proof of existing marriage.
  • There is no 72-hour waiting period before the license is issued

 WHAT ARE THE RESPONSIBILITIES OF A COUPLE FOLLOWING THEIR MARRIAGE/CIVIL UNION CEREMONY

  • Ensure that the marriage license has been filed with the Registrar of the municipality in which the ceremony was performed.
  • Ten -Fourteen (10-14) days after the ceremony, the couple can request a certified copy of the marriage license with the registrar of the municipality in which the ceremony was performed. The requirements for applying for certified copies are discussed below.

 

CERTIFIED COPIES OF A VITAL RECORD

This office, upon request, supplies certified copies of non-genealogical vital records of all births, deaths and marriage/civil unions where the event took place in Bridgewater Township. Each copy is $25.00 and $2.00 for each additional certified copy. Checks/money orders should be made out to Bridgewater Township. We do not accept credit/debit cards.

Additionally, applications for corrections to vital records may be requested through this office if the event occurred in Bridgewater Township.

WHERE, WHEN & HOW DO I GET A COPY OF A VITAL RECORD?

  • You may apply to receive a certified copy of a vital record ONLY if the event occurred in Bridgewater Township.
  • Proof of identity is required. Acceptable forms of ID include Valid Driver’s License/ Passport/ State/Federal ID
  • To request a certified copy of a vital record by mail, fill out an application, photocopy your driver’s license with your current address and include $25.00 (check, money order) for first copy and $2.00 for each additional certified copy. Include a stamped self-addressed envelope and mail to:

WHAT IS THE PRICE FOR A VITAL RECORD?
All certified copies of vital records are $25.00 each for the first certified copy and $2.00 for each additional certified copy.

 

DOMESTIC PARTNERSHIPS

WHAT IS A DOMESTIC PARTNERSHIP AND HOW CAN I REGISTER FOR ONE?

Couples wishing to register a Domestic Partnership must be same sex couples OR opposite sex couples who are both 62 years of age or older. Couples must meet certain requirements.

The Affidavit of Domestic Partnership is not available as an electronic form. It is a carbon copy form that must be obtained from our offices.
